The spring 2026 Leapfrog Hospital Safety Grade Courtesy Review Period will be open from February 24 to March 17. Hospitals will be able to access the Review Website to:

• Confirm your hospital’s name, address, and CMS Certification Number.
• Review the source data used in the spring 2026 Leapfrog Hospital Safety Grade.
• Provide contact information for your hospital so we can send you important announcements about the Leapfrog Hospital Safety Grade.

Hospitals with enough available data to receive a Leapfrog Hospital Safety Grade will also have access to the Leapfrog Hospital Safety Grade Calculator© and instructions for validating their hospital’s data. Note that the numerical score available on the final page is preliminary and subject to change during the Letter Grade Embargo Period. No letter grades have been assigned yet.

The Letter Grade Embargo Period will take place in April. During the Letter Grade Embargo Period, we will refresh this website to reflect any changes that occurred during the Review Period and display finalized numerical scores and hospital letter grades. A full timeline is available at https://www.hospitalsafetygrade.org/for-hospitals/key-dates-and-information.
